INJURED ON THE JOB?

Injured at work, you need to contact a manager and fill out an accident report.

Keep a note of the date of incident, time, location (Gate number, specific doorway in the building, lunchroom, etc.) and name and contact of the eyewitness (crew, co-worker, managers, store clerk, passengers, etc.). If applicable also note the weather conditions if you’re outside and the type of equipment you used (Dane, type of wheelchair, etc.).

It is recommended you see your Doctor immediately. If your Doctor recommend you not return to work:

A) Your Doctor’s office will fill out a FORM 8 and they will submit this Form to WSIB.

NOTE: If you visit the hospital, advise the injury is work related and ask them to fill in a FORM 8.

B) Employee will need to fill in an Online FORM 6 at www.wsib.ca/reporting. This online submission will be sent directly to WSIB.

C) Air Canada’s Disability Manager (also known as Attendance Manager) will use the injury/accident report to fill out a FORM 7 and they will submit this Form to WSIB. If you were unable to fill out a report because you left work immediately to get treatment and have not come back to work since, please contact the H&S team at safety@yyzd301.com | 905-676-2992 as soon as possible.

* Should you return to work and there was no time lost from work, the incident is logged for statistical purposes.

REOCCURANCE OF INJURY OR ILLNESS AND UNABLE TO WORK:

If the employee return to works but the injury or illness persist and they are unable to work.

The employee must;

1. Reach out to Cynthia Loong and she will provide the contact information to Disability Management to file a reoccurrence injury report

2. Visit your doctor fill in a new Form 8

3. Reapply for WSIB and reference the previous Claim File Number on the new submission.
