- Services of a licensed/registered chiropractor, naturopath, osteopath and podiatrist /chiropodist
$50 per visit to a maximum of $1,000 per person or $2,000 per family, per calendar year - Services of a licensed/registered massage therapist
$80 per visit to a maximum $800 per family, per calendar year - Eye examinations (Provided by a licensed ophthalmologist or optometrist)
One every 24 consecutive months to a maximum of $125 - Frames and prescription lenses or prescription contact lenses, prescription sunglasses, safety glasses and goggles when dispensed by a licensed optometrist, optician or ophthalmologist.
• $350 every twenty-four (24) consecutive months per employee and eligible spouse from last date of purchase.
• $325 every twenty-four (24) consecutive months per eligible dependent child.
NOTE: If this benefit is claimed no other laser eye surgery will be eligible for reimbursement until after the 24-month period has elapsed. $600 per person every 48 consecutive months for laser eye surgery. If this benefit is claimed no other vision care (glasses/contact lenses) coverage will be eligible for reimbursement until after the 48-month period has elapsed.
